Patient Family Advisory Council

A Patient & Family Advisory Council (PFAC) partners patients and families with members of the healthcare team to provide guidance on how to improve the patient and family experience.

Patients and their families are often the most knowledgeable member of the care team and can offer a unique perspective and valuable feedback regarding care they receive. The CHS advisors provide insight across a variety of medical services and disciplines.

Our mission is “Past and present patients, family members and CHS staff working together to improve the patient and family experience at Coffey Health System.”

The council supports CHS through four primary initiatives:

  • Identify patient and family needs and concerns,
  • Provide feedback on our current systems and process within the health system,
  • Generate new ideas to improve the quality of patient care, and actively helps implement change, and
  • Act as catalysts and advocate to integrate patient and family-centered care across the health system.

 

Participants serve a one-year commitment, which involves a training session and two to four meetings.

If the following qualities describe you, please complete the introduction form (below).

  • Age 18 or older
  • Past or present patients and family members,
  • Share insights and information about experiences in ways that others can learn from,
  • Shows concern and interest for more than one issue or agenda,
  • Respect the perspectives of others,
  • Speak comfortably in a small group setting,
  • Work in partnership with others, and
  • Invested commitment to PFAC.
